the subject's doctor, ALPHONSO A, LOMBARDI, to^
that the subject was "a sick man" and would be in the

advised that during the INS hearing on November 3>

NY 1/26/59 and then as of 3/7/59 resided at the Golden Gate
Hotel, Miami Beach, prior to returning to NYC 3/26/59.
INS, NYC, advised that GAMBINO 'S deportation hearing
is still pending because of his heart condition. They advise
there is no Indication that GAMBINO Is active in the underworld
because o£ his health.
UACB, because of GAMBINO' S health and reported in-
activity, this case is being placed in a pending Inactive status.
Contact is being made with INS on a continuing basis
re GAMBINO. If in the future, information comes to the attention

reportedly obtained on November 25, 1957, a warrant for the
arrest of Carlo Gamblno charging hln vlth violation of
Section 241A1 of INS Act of December 29, 1952, with an
order to 'show cause vby he should not be deported and
charging Gamblno as being a National of Italy who entered
the United States on December 23, 1921, at Norfolk,
Virginia, as a stowaway on the **SS Vlncenzo Florlo." (Subject's

G£uablno was served with a deportation warrant oa
August 6, 1958, and posted a $10,000 bond. He was released
until September 3, 1958, when INS scheduled a hearing, but
on that day the hearing was postponed because of Ganblno *s
health, (ins)
. An article Entitled "Apalachln Figure ^jjs 11^/4^
Bearing," appearing lu the "New York Times" of November 4,
1958, reports that Carlo Gumblno was taken ill at the start
of his INS deportation hearing, November 3, 1958. The article
relates that Gamblno was taken In a wheel chair firon the
bearing room at INS, 70 Columbus Avenue, New York City, to
an ambulance and was subsequently admitted to the
Flower-Fifth Avenue Hospital, Now York City, where he had
previously been treated for heart trouble.
The "Now York Daily Ulrror" of November 15, 1957,
contains an article wherein it states that "62 top leaders
of the dreaded Uafia, ruling crime syndicate of the US, were
on November 14, 1957, during a top-echelon conclave called
to deal with mounting official pressure on their lucrative
strangle hold on the natlon^S puree.*' According to the article,
the meeting was held at /palachin. New York, and Gamblno
was present at this meeting.

Mr, STRAPP advised that on July 29, 1935, GAMBINO
was granted a pre-exainlner*s hearing at the United States
Consulate, Montreal, Canada, and was refused a visa. GAMBINO
re-entered the United States at Rouses Point, NevT York,
on July 29, 1933. He was pre-examlned by a Board of Inquiry
at Rouses Point j New York, and re-entry was granted.
According to Mr. STRAPP, a warrant of arrest was issued
on November 13, 1936. against GAMBINO, charging entry at a port
subsequent to July 2o, 1933, at which time he was not In
possession of a visa. On January 2L, 1937, the warrant which
was Issued on November 13, 1936, was cancelled by an authorized
officer.

Mr. STRAPP Informed that the INS obtained on
November 25, 1957* & warrant for the arrest of CARLO aAMBiNO

Italy, who entered the United States on December 23, 1921, at

Mr. STRAPP stated that INS warrant has not been
served because subject GAMBINO 18 presently in the Flower
Hospital, 5th Avenue and 105th Street, New York City, for a
heart operation.
Mr, STRAPP informed that QAMKLNO is registered under
Alien Registration number A 5451526.
On August 7, 1958,|^HHH||B Investigative

^that subject, accompanied by his attorney
, 111 Broadway, New York City, accepted service
of a deportation warrant and an order to show cause on
August 6, 195d. Subject posted a $10,000.00 bond and was
released on ball until September 3, I958, when INS had
scheduled a hearing.
On SeptemberlT^QSd, |^|^HH^ Investigative
Unit, INS, advised SA^^^^^ tJlSMWTOf^ith his
attorneys, ABRAHAM BROTSR^nST EDWARD ENNIS, appeSred at the
INS Headquarters on September 3, I958, with a medical
certificate from Dr. ALPHONSO A. LOMBARDI, 101 East 105th
Street, New York City. This certificate refl^tgdthat
QAMBINO was suffering from a heart condition J^^^^p stated
that because of this medical certificate, INSH^Rponed the
hearing until October 2, 1958.
On August 16, 1S^6(^^^^^^^H INS,
20 West Broadway, New York, advised SA VHHP^IHb^^^^
the subject entered the United States Public He^reiniospital
at Stapleton, Staten Island, to determine his ability to
physically withstand the rigors of a deportation hearing.

Hearing", appearing in the "New York Times" of November 4,
1958* reports that CARLO GAMBINO was taken 111 at the start
of his INS deportation hearing, November 3, 1958. The

' Narcotics, testified before the New York State Joint legis-
lative Conanittee on Government Operations on January 9» 195v,
at Albany, New York, regarding the meeting held at Apalachin,
New York, on November 14, 1957. He testified that the
Bureau of Narcotics became particularly interested in this
meeting as reports from Apalachln Indicated that at least
• three narcotic violators of major Impvx^ance were present.

have long been known to the BureaCT of Narcotics as
important leaders in the Kafia and intimate assoclatei
of SALVATORE LUCANIA, with alias, •Iwcky* Luciano,
''During the spring of 19^8, reliable infor-
mation obtained from a Bureau of Narcotics source
indicated that CARLO GAMBINO travelled clandestinely
to Palermo, Sicily, where he Joined his brother,
PAUL (GAMBINO), who had fled to Italy to avoid pros-
ecution in a Federal alcohol tax ease. The GAMBINOS
were reported to exercise control of the narcotic
smuggling activities between the Mafia element in
Palermo and the United states on behalf of SALVATORE
LUCANIA and during 19^8, both OAMBINO brothers met
with LUCANIA at te home of their relatives in Palenr
Sicily,

isted for violation of Internal Revenue Laws
of the United States involving the sale of non-tax paid
alcohol.
The subject was arrested with nine other
individuals and their Criminal Docket Number 7607, In
United States District Court, Federal Building, New York
City, i. . reflected that the subject was tried, found
guilty May 1, 1939. On January 31,. 19^0, United States
Court of Appeals reversed the Judgment and ordered a new
trial.
On June S, 19^1, subject was again tried, was
found guilty on June 12, 19^1, sentenced to pay a fine
of $1,000.00, sentence of one year suspended, probation
five years June I7, 19^1. The sentence was amended
for Imprisonment of term eleven months and 29 days
suspended.
The files of ATTU, New York City, as reviewed
In December, 1957* under case number 4215M, reflected
that the subject and 29 others were indicted ih ,}^he Eastezn
District of New York, In connection with a bootleg alcohol
operation. The indictments were subsequently dismissed
on February 7« 19^0, when It was conceded by the United
States Attorney that the indictments were based on wire
tapping evidence.

District of New York, In which the United States was
suing the subject and others to recover over a nllllon '

Part Examined: Carvlcal Spine November 28, i960
Examination of the carvical spine was made in multiple
projections* There is noted diffuse hypertropic changes
throustiout tne carvlcal spine, particularly at the level
of trie sixth and seventa carvical vertebrae • The
intervartebral disc spaces are normal, as well as the
interpedicular distances. There is no other evidence of
osteal or periosteal patnology. There is noted some
straightening of the normal carvical lordosis ♦
Electrocardiogram : ' November 2, I96O
Sinus rhythm, rate 74 •
Grade I AV Block: PR-2 equals 0.24 sec.
HiG^nt BB Block of broad SI type with some xlght axis
deviation. There are typical changes including evidence
of late activation in tiie caest leads on the right.
Also the R waves become smaller from V2 to V3 till
finally they are quite small and low in position in
This would suggest ischemic damage which is presumably
interior but an old condition.
Evidence of myocardial abnormality: 2 plus to 3 plus.

In ttiC earlier record tnere is a slight degree of Grade I
AV BlociC ..loh PR-2 averaging 0^,24 sec* Today I find
one complex with a PR Interval as short as 0,20 sec*
but others vary up to 0*2b sec. and seem to average
0*22 sec, I believe that AV conduction Is subnoi-mal and
the condition is about the same*
Again there is ricnt BB Block of br-oad Si type with some
right axis deviation and again these features seem about
the same, as they were before. Again there are low R waves
in V4 particularly which strangly suggest a coronary
background as I do not believe these can be explained as
incidental to the zone of transition altnough that could
be a factor* The patient is getting digitalis but I
do not note any cnanges due to the drug Which can be
Identified specifically.
Evidence of myocardial abnormality: 2 plus to 3 plus.
Infarction: A coronary background is probably as noted
before but I am unable to see arranges which might suggest
an active process* However we must remember thax^ ajiy
foi^m of BB Block might mask such a process.

There Is a moderate degree of right axis deviation*
Heart block of 2 kinds is present:
Grade 1 AV Block as PR-2 equals 0» 22-0, 24 sec. This
might- be due In part to digitalis but has been present
earlier and sup,ce3ts some damage in the septal region.
Rlgiit DB Block" of broad Si type was first recorded in
this scries of records on July 12, 19^0, The preceedlng
record of Marcn 2, I960 did not slow right BB Block,
However the previous records do give evidence of
arteriosclerotic heart disease with coronary changes and
a diagnosis of antaroseptal infarction was made at
one time in 1959.

Some damage in tne septal region is probably Involving
both the main stem and the Right BB Branch. However the
presence of right axis deviation with a right BB
Block is further abnormalit:/ and might be evidence of
some tight sided strain. As compared to November 3, 196O:
I believe tnere is a little more sagging of the RS^-T
segment in II and again in AVP which presumably is a
digitalis effect. Chest leads show minor differences
but the overall pattern is about the same.
Evidence of myocardial abnormality: 2 plus - 3 plus:
Heart block of two types due to degenerative arteriosclerotic
diseases including coronary changes^

Sinus rhythm rate 66-70 •
Tnere is no definite axi:^. deviation.
Grade I AV Block: PR-2 cauals 0,22 sec*
Kight BB Block of broad type.
Some evidence of a digitalis effect, presumably
responsible for instance:: for the lowering of RS-T
in II. Also the drug probably makes some of the T
waves a little smaller tnan they otherwise would be«
Chest leads from thie rignt show the characteristic
evidence of late activation of the right ventricle. Beyond
this the chest leads" on the left show a little more
depression of HS-T and T in V5 and V6 which is presumably
a drug effect.
Infarction: A coronary background would appear probably
but cannot be proven by this record.
Evidence of mi^ocardial abnormality: 2 plus - 3 plus.
Electrocardiogram : November 22, i960
Sinus rhythm, rate 90.
AV conduction is normal; PR-2 equals 0.20 sec. wnich may
be relatively long for this dominant rate but still Is
technically within the normal range.
There is right axis deviation with right BB Block of
broad SI type.

As compared to the last record of November I6, I96O:
In the standard limb leads the R wave is smaller in II
and the S wave broader wltn a different type of slurring
and lowering of the RS-T segments. However, the patient
is getting digitalis which might account for this RS-T
depression. Two ventricular extra-systoles are recorded
in III.
The augmented limb leads do not show any new change
except in AVF where the QRS group is smaller and the
RS-T segment still depressed and here again we may suspect
digitalis.
In the chest leads we do note changes with more definite
elevation of B3-T in V2 and V3 although in V3 the T
wave is more definitely upright. Beyond this the
broad S vaves are seen in and V6 but are simply
characteristic of the right BB Block. Depression of
RS-T in V6 may again be due to digitalis.
1. The BB Block in this case developed some time between
March 2nd, 19^0 and July 22, 1960 and this deve;Lopment
during a period of observation is strong evidence of
degenerative heart disease and not the sim.ple benign
type of Right BB Block which we find in routine health
examinations in subjects otnerwise apparently normal.
2. Depression of the T waves in chest leads from the
last is of course incidental to the BB Block of this type*
'Also elevation of RS-t seems to have been a fairly
constant feature in V2 and V3 and this series and is not
a fresh development since November 16, i960 although
it seems to be getting more marked. This has a rather
coronary appearance and might point to subandocardial
or anterior Ischemic damage but as it is not entirely
a new finding, we must be cautious in assigning
signil'icance to it.
3. Tiic low RS-T segment would rather suggest coronary
insufficiency or subandocardial iscxiemia but here we are
dealing wlx.h digitalis administration which could well
account for tnls change.

slurred wltn lower ajnplitude In some leads and this
gives the impression oT an acute degenerated change.
Nothinc is entirely typical because largely of the
BB Block but an acute process which probably is chiefly
anterior Is suggested.
Evidence of myocardial abnormality: 3 plus to 4 plus*
Infarction: An acute coronary process would appear
quite possible if not probable and may well be anterior
in location*

Grade LAV Block: PR-2 equals 0,23 sec.
Tnere is a little right axis deviation.
Tne overall appearance of this record l3 fairly
similar to the last one of November 22nd and Includes
particularly the changes due to right BB Block of
broad ol type. At present we may note the following:
The PR interval was normil on November 22nd and is now
prolonged up to 0,23 sec,^ a slight degree of Grade I
AV Block. However PR time of 0.22 sec. was noted
on November Ibtn, In any event this could well be a
Agitalls effect but the patient is getting more
digitalis day by day and actually prolongation of PR
up to 0.24 sec. was seen earlier in this series.
Therefore a damage In the septal region would appear
probable.
Today depression of RS-T in II seems less marked although
this was apparently a drug effect and would be expected
to continue.
In the Ghent leads from the right side from the
ciiarac teristic chan^c^s due to ri^ht BB Block we note
that less elevation of Ro-T especially In V2 but also in
V3. This would suggest that we are dealing with a
coronai^y Ciiange that is improving, presumably some
anterior or subandocardial Ischemia. Beyond this the T
waves are rather smaller than they were In V5 and V6 with
a low RS-T in v6 but here again of course digitalis
would account for what wc see.

We are dealing with degenerative heart disease including:
A* Tae appearance of right BB Block under observation
as noted before.
B. Cnanccs in AV conduction which seem to vary although
- digitalis has been given constantly and therefore
some dajnage of the septal region would appear
probable.
C. Chainges in the R3-T level In chest leads from the
rignt v/hicn hardly can be accounted for by other
conditions although digitalis might confuse the
issue here but some anterior Ischemic condition would
appear probaole.
A drug effect which in general Is rather less now
than before except in V5 axid V6.
Elect rocardiograjn ; December 5, I96O
Sinus rnytiim, rate 72. Grade I AV Block: PR-2 equals
0.22 - 0.24 sec.
There is a tendency to rignt axis deviation.
As compared to November 2b th, 19^0 •
In tne stsoidard limb Jeads we still see the broad S waves
in I and II related to the right BB Block observed before.
In the augmented limb leads the general contour Is the
same as it was.
In tae chest leads we have a typical of late activation
in VI and beyond this. There is still some elevation
of RS-T in V2 and even in V3 but less than it was on
November 22nd, and here I suspect that we are dealing
with a coronary change. Vlhether or not It has oome
dov/n as the result of digitalis or natural evolution
cannot of course be determined but as noted, I have
suspected some Ischemic dajnage anterior in location.
Beyond 4;hl3 the chest leads seem to be fairly similar
to what they were and the same remarks apply to th3s
record as noted in regard to the preceeding one.
Evidence of myocardial abnormality: 2 plus to 3 plus.

one hour 10 min.
The records further reflected that on November 1, I960,
Mr. GAMblNO was readmitted into the Flower and Fifth
Avenue Hospital because of chest pain. A rather extensive
study was done to determine the etiology of this chest
pain arid serial electrocardiograms demonstrated definite
Impairment to the circulation in his heart with progressive
damage to tiie muscles of his heart indicating degeneration
in his general cardiac status. Tne patient was treated
in the- same fashion that one would treat an acute

for cardiac evaluation on August 18, 19^0. The
report reflects the following:
Diagnoses in this case are:
1. Arteriosclerotic heart disease
2. Congestive heart failure, compensated
at preseuo
Axigina pectoris syndrome
Diabetes mellitus, controlled by diet
Regarding your questions about wi^ether Mr* GAMBINO
can take part in deportation proceedings; it is our.
opinion that he may^be served papers and interrogated
witacNat much risk to the patient. It is possible that
under the condition of emotional excitement, Mr.
GAMBiriO will have episodes of angina pectoris pain.
These can be readily controlled by the patient taking
nitroglycerin tablets. We suggest tnat the hearings
be neid as informally as possible and that the questioning
pei'lod be no more than 20 to 30 minutes in lengtn with
a siiort rest interval between each questioning period,
ir Mr, GAMBINO would ta.:c a nitroglycerin prior to the
questioning period he may have less difficulty with his
anginal pain. A very small risk does exist that if the
patient were to become severely agitated, he might
have a myocardial infarction.

LOr^BARDI^s statement in part is as follows:
Considering Mr^ GAMBINO •s generally poor
condition^ I would strongly advise against any such
readmlssion and reinvestigation. As you know, he
apparently was thoroughly Investigated for quite some
time at the Public Health Hospital only recently in
August^ 19o0. After his discharge from the hospital
after that investigation, his chest pain, cardiac status
and general condition worsened to a degree requiring
furtner^ hospitalization soon after, under my care,
at the Flower and Fifth Avenue Hospital for a period of
almost two months until December, I96O.
For a time during this cardiac attack, the
outcomo as far as his lire was concerned was in doubt.
Mi' previous reports to you nave indicated that his
prognosis is poor and his life expectancy la indeed
very limited. His poor condition can be easily affected
by any emotion or physical strain.
It is my opinion that any such readmlssion
for reinvestigation ^should be avoided if at all possible,
as it would increase his anxiety and possibly cause
further physical deterioration or harm.
